Carolina Childcare Licensing Rules summary for family
home childcare providers reflects the N.C. SIDS law and
addresses infant sleep safety and SIDS risk reduction.
The safe sleep rules went into effect May 1, 2004. They
apply to childcare providers licensed to care for infants
12 months of age or younger.

Waiver
Some infants may be exempt from the back sleep position
requirement because of a medical condition. In this case,
the Alternative
Sleep Position Waiver - Physician Recommendation (medical
waiver) may apply.
Follow these guidelines when using a medical waiver:
- The childcare provider should give the parents the official
DCD form to be completed by the child's doctor.
- The medical waiver must be signed by the child's primary
physician.
- The medical condition must be stated.
- The doctor's contact information and signature must
be included.
- The parents should return the completed form to the
childcare provider.
- A copy of the medical waiver will be kept in the child's
file.
- A note indicating the medical waiver is in use and its
location must be posted next to the infant's crib.
