Now available! Updated Health Check/NC Health Choice fact sheets and envelope stuffers with income guidelines for 2009-2010

(3/09) - New bilingual Health Check/NC Health Choice fact sheets and envelope stuffers with income guidelines effective April 1, 2009 through March 31, 2010, are now available from the North Carolina Healthy Start Foundation!

The envelope stuffer has a new look to match the fact sheet and the new poster that is in design. Please take note that the TTY for hearing impaired phone number has been removed from the fact sheet because the line is no longer in service.  Deaf and hard of hearing persons may contact the North Carolina Family Health Resource Line using the assistance of a relay operator by dialing 711.

Please remember to replace your ‘old’ fact sheets and envelope stuffers.  Fact sheets come in sets of 100 -- either padded (item # D4) or folded as brochures (item # D4Br).
