What Community Health Coaches Do
Your role as a Community Health Coach would be to raise awareness of health challenges and health resources in your community. As a Community Health Coach, you will play an active and important role in helping people in your community think about and develop a plan for their future and the future of their family.
It is easier than you think. As a Community Health Coach you do not need to be a "health and wellness expert!"
You will receive all the materials you need to be successful:
- Free training
- Free materials, resources and great incentives
- On-going support
The Community Health Coach trainings will be offered to people who live or work in Edgecombe, Gates, Halifax, Hertford, Nash and Northampton counties. Training sessions are offered using Ready, Set, Plan!, a training program designed to reach community-based and faith-based organizations.
Which training is right for you?
- Community-based organizations will be trained on Ready, Set, Plan! as a Community Health Coach, you can provide this life changing information during counseling sessions, one-on-one with individual clients and in your regular work setting.
- Pastors and members of faith-based organizations will be trained on Ready, Set, Plan! also. As a Community Health Coach, you can provide this life changing information through Bible study classes, sermons, marriage counseling, youth ministries and men's and women's ministries. Trainings for faith-based organizations will be announced soon. When the training dates are confirmed we will post them on our web site.

Ready, Set, Plan!

Ready, Set, Plan! is an opportunity to bring people together to talk about family health in community-based and faith-based conversations. It combines important health information with engaging activities. It challenges the participants to apply the curriculum to their everyday lives. Ready, Set, Plan! aims to connect a host of community partners including: healthcare providers, community-based organizations, individuals, local activists and other small groups. Through these connections, families in northeastern North Carolina will be better equipped to begin planning a healthy family and a healthy future.
Ready, Set, Plan! is also referred to as RSP.
Ready, Set, Plan! (RSP) focuses on general, emotional and reproductive health. Ready, Set, Plan! includes a RSP kit equipped with a Coach's Manual, a Tool Box with health resources to share and a Routes Flip Chart that can help guide conversations around "Are You Ready?" Do you have a plan to help create a healthier life?
Ready, Set, Plan! Routes
Ready, Set, Plan! takes three routes to a healthier life and a healthier family.
- Health and Wellness Route
- Emotional Health and Wellness Route
- Planning a Family Route
All of the routes share the same goal of bringing people together to talk about the importance of family health and family planning in community-based conversations. The routes help people make choices about how to have a healthy life and a healthy family.
All of the Routes ask the questions "Are You Ready?" and "What's Your Plan?" The "Are You Ready?" questions focus on choices people make that impact their health and the health of their families. The "What's Your Plan?" questions prompt people to make decisions on what they will do to be healthier.
